29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Second floor top plate, new residential construction located at 1200 Lee Street, Milton, GA 30004: Employees were exposed to fall hazards of approximately 22-feet, 4-inches while installing roof trusses on the top plate with no fall protection system in place.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) East side, new residential construction located at 1200 Lee Street, Milton, GA 30004: Employee was exposed to impalement hazards while working on the top plate directly above unprotected rebar which was protruding 2-feet, 6-inches above the concrete foundation below.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Temporary service box on the west side, new residential construction located at 1200 Lee Street, Milton, GA 30004: The employer failed to provide flexible cords which were free from damage. The energized internal circuits located within the protective casing of the 10-gauge cord had pulled away from the male and female ends of the cord as the strain relief was damaged.
