(a) Martinsburg: EverGuard Bonding Adhesive, an eye irritant that contains toluene and acetone was being used by an employee without eye or face protection while gluing down a rubber membrane to the edges of the flat roof, as determined on or about October 14, 2015 and times prior. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM.

ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Martinsburg: Employees were applying a rubber membrane to a 17,200 square foot flat roof, working on the roof edge, outside of the warning line without the use of a personal fall arrest system, as observed on October 14, 2015 and times prior. ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQURED FOR THIS ITEM.

29 CFR 1926.502(h)(1): The employer shall designate a competent person to monitor the safety of other employees and the employer shall ensure that the safety monitor complies with the following requirements: (i) The safety monitor shall be competent to recognize fall hazards; (ii) The safety monitor shall warn the employee when it appears that the employee is unaware of a fall hazard or is acting in an unsafe manner; (iii) The safety monitor shall be on the same walking/working surface and within visual sighting distance of the employee being monitored; (iv) The safety monitor shall be close enough to communicate orally with the employee; and (v) The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function. (a) Martinsburg: Employees were applying a rubber membrane to a 17,200 square foot flat roof, working on the roof edge, outside of the warning line and the safety monitor was not within visual distance and was not close enough to communicate with the employees, as observed on October 14, 2015 and times prior. ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQURED FOR THIS ITEM.
