29 CFR 1926.405(g)(1)(ii): Flexible cord was not equipped with an attachment plug and energized from a receptacle outlet when used as permitted in 29 CFR 1926.405(g)(1)(i) [C], [F], or [H]:  On or about October 15, 2015 at the residential job site located at 3133 Grand Way Ave, Baton Rouge, Louisiana the employer allowed an employee to insert wires which were attached to the male attachment plug into the 110 volt side of a 220 volt outlet.
