29 CFR 1926.1060(a): The employer did not provide a training program for each employee and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. On or about October 16th, 2015, at the jobsite located at 1330 S Ocean Drive, Palm Beach, FL 33480, an employee did not receive training about good safe practices while using ladders, and fell from approximately 12 feet height while working on a frame ladder.
