29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.    On or about October 19, 2015, the employer allowed employees to use an all terrain fork lift with a pallet on the forks as a scaffold platform.  The platform was not securely attached to the forklift.  The employee was exposed to a fall of a least 10 feet to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about October 19, 2015, employer did not provide fall protection to each employee working on a scaffold platform.  The employee was exposed to a fall of over 10 feet to the ground below.
