29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Roof, new residential construction located at 3083 Staglin Drive, Powder Springs, GA 30127: Employee was exposed to falls of approximately 20-feet, 4-inches to the lower level while installing 15# felt on a 8/12 pitched roof with no connection to the life-line installed on the ridge. The employee was not using a complete personal fall arrest system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Front side, new residential construction located at 3083 Staglin Drive, Powder Springs, GA 30127: The ladder provided by the employer was not secured to the structure or extended at least 3-feet past the eave to allow for safe access on and off the roof. Employees were exposed to falls of approximately 11-feet, 1-inch to the lower level.
