29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress.    On or about October 21, 2015, while performing masonry activities on a job site located at 14237 Athens Ave., Lakewood, Ohio 44107, the employer failed to protect employees from a fall of approximately 14 feet when employees used frame rails and cross braces to access the working level of the supported scaffold.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    On or about October 21, 2015, while performing masonry activities on a job site located at 14237 Athens Ave., Lakewood, Ohio 44107, the employer failed to protect employees from an approximate fall of 14 feet when mid rails and end protection were not installed on the supported scaffold.
