29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Wally Pontiff Jr. Playground, Metairie, LA; On or about October 20, 2015, the employer allowed an employee to work from an aerial lift while not wearing fall protection. The employee was installing metal soffit and fascia to the exterior of a building. The employee was approximately 22 feet above the ground.
