In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided or used:    a) 1815 Landmeir Rd.- On or about October 22, 2015, the employer did not provide and require the use of personal protective equipment, including NFPA 70E arc-rated protection for the eyes, face, head, extremities, and protective clothing, when employees performed work on live electrical equipment or any equipment that had the potential to become live, such as the 480 Volt panel on the northeast wall of the Warehouse Area.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) 1815 Landmeir Rd.- On or about October 22, 2015, the employer did not ensure that employees were protected against electric shock when working on or near electric power circuits, such as the 480 Volt panel on the northeast wall of the Warehouse area, by de-energizing and grounding electrical equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
