29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):       On or about October 8, 2015, on the steep roof of a single family dwelling employees were exposed to a fall hazard exceeding six feet when performing roofing work without a fall protection system.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  This violation occurred on or about October 9, 2015, where an employee fell from a roof of a residential home and was hospitalized as a result of a work-related injury that was not reported to OSHA within 24 hours.
