(a) Location - Shop Area: On or about 10/23/15, the employer failed to ensure an exit route was free from obstructions.     Alvelo Enterprises, LLC, dba Carpenter-Costin Co was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910. 36(g)(4) (a) which was contained in OSHA Inspection Number 964745 Citation Number 01, Item Number 1,  Issued 7/1/14 with respect to workplace located at 235 Burrill Street. Swampscott, MA 01907 The final order date for the citation was on 7/24/14.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire;     (a) Location - Shop area:  On or about 10/23/15, employees were exposed to burns when oxygen cylinders were not separated from fuel-gas cylinders.      Alvelo Enterprises, LLC, dba Carpenter-Costin Co was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910. 253(b)(4)(i) which was contained in OSHA Inspection Number 964745 Citation Number 01, Item Number 4,  Issued 7/1/14 with respect to workplace located at 235 Burrill Street.
