29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 21, 2015, and at times prior thereto, the employer did not ensure the employees operating heavy machinery were trained to recognize and avoid unsafe conditions while working around electrical power lines, exposing employees to electrical shock or electrocution hazards.

29 CFR 1926.600(a)(6)(ii): For lines rated over 50 kV, a minimum of 10 feet plus 0.4 inch for each 1kV over 50 kV was not maintained between the energized line and the equipment. On or about October 21, 2015, and at times prior thereto, the employer did not ensure employees operating heavy equipment around overhead power lines maintained a clearance distance of at least 10 feet 7 inches, exposing employees to electrical shock and electrocution hazards.
