29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about October 22, 2015, during the installation of roofing material at 410 Cayuga Ave NW, Canton, Ohio 44708; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces with a height of approximately 12.5 feet.
