29 CFR 1910.134(c)(2)(ii): In addition, the employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks). Location: Workshop Employees were exposed to respiratory related hazards while voluntarily using tight-fitting, half-face respirators and dust masks where medical evaluations had not been conducted. The Master Millworks, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(ii), which was contained in OSHA inspection number 610178.015, citation number 1, item number 1 and was affirmed as a final order on 10/11/2012, with respect to a workplace located at 2 Bridge Approach Road, Buzzards Bay, MA.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Location: Workshop The employer did not notify the Department of Labor, Occupational Safety and Health Administration within 24 hours of an employee's left ring finger amputation injury and in-patient hospitalization for surgery.
