29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.    (a) Location - 465 Washington Street, Winchester, MA: 2nd floor.  On or about 10/27/2015, employees working adjacent to a window opening that was 24 feet above the ground were not protected from falling by means of a top rail.    (b) Location - 465 Washington Street, Winchester, MA: 1st floor.  On or about 10/27/2015, the employer did not cover open holes on a job site that exposed employees to a fall of 8' - 10 1/2 inches.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    (a) Location - 465 Washington Street, Winchester, MA: 2nd floor.  On or about 10/27/2015, employees using a stairwell that did not have a stair rail system installed to protect employees.
