29 CFR 1926.95(a): At the job site, personal protective equipment was not maintained in a sanitary and reliable condition when hazards capable of causing injury or impairment were encountered:   (a) Navy Base Exchange, Navy Lodge Bldg. 3526:  One Husqvama Brand protective helmet with ear and face protection and one ear muff Model M2 were not maintained and stored in a sanitary and reliable condition. They were stored next to a MK 5000 Block saw and actively collected concrete debris.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:  (a) Navy Base Exchange, Navy Lodge Bldg. 3526:  Two yellow extension cords were not protected from damage by moving forklift at the worksite. The cords were lying on the ground and heavy equipment ran over the cords throughout the work shift.
