29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 20, 2015 at 1049 Stevens Creek Road, Augusta, GA., four (4) employees working on removing and installing new roofing sheathing and shingles on the Homewood Suites by Hilton, were exposed to a fall hazard of approximately fifty (50) feet to the lower level when they were wearing their fall protection, but not tied off in such a manner that would prevent them from contacting the lower level.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:Note: Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; or inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. On or about October 20, 2015 at 1049 Stevens Creek Road, Augusta, Ga., where four (4) employees were exposed to a fall hazard of approximately 50 feet to the lower level. The employees were wearing but not utilizing their fall protection showing a lack of understanding of the need to be protected from falling to the lower level.
