29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    (a) Frequent and regular inspections of the job site, materials and equipment to be made by a competent person were not made as indicated by:    1.     Roofing work occurred without fall protection at the western edge of the structure.      2.     Flagging installed at the eastern edge of the structure was installed at an improper height and distance.    3.    Parapet walls used for fall protection were less than 2 feet in height.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      (a)  The training program did not enable workers to:    1.  Recognize the hazards of falling and the procedures to be followed in order to minimize fall hazards, as proper       procedures were not followed.    2.  Understand the correct procedures for erecting the fall  protection used, as proper procedures were not followed.           3.  Understand when the use of guardrail systems, personal fall arrest systems, safety net systems, or safety monitoring       systems are required, as proper procedures were not followed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were   employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a) At the 6204 S. Kingshighway work site workers were performing roofing activities at a height of approximately 25 feet without fall protection.    Petko's Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 280180 Citation number 2,   Item 1, and was affirmed as a final order on November 8, 2012 with respect to a workplace located at 4340 McRee Ave. St Louis Mo, 63118.    Petko's Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 314281783 Citation number 1, Item 3 and was affirmed as a final order on August 27, 2010 wiith respect to a workplace located at 2220 Oregon Street, St. Louis Mo 63104.
