29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  On the roof of a commercial building, west side, on or about October 29, 2015, employee(s) without fall protection were exposed to fall hazards greater than 6 feet to the paved ground below, while replacing the surface of a flat roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On the roof of a commercial building, west side, on or about October 29, 2015, employees without fall protection were exposed to a fall hazards of 14 feet to the paved ground below, while not being trained on the procedures to recognize fall hazards.

29 CFR 1926.1053(b)(1): When a portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:    On the roof of a commercial building, west side, on or about October 29, 2015, employees were exposed to fall hazards of approximately 14 feet to the paved ground below, when using a ladder that was approximately 1 foot above the upper landing surface, while gaining access to a roof.
