29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) A). On or about October 28, 2015 Employees were working off the top section of a metal tubular scaffold system while setting roof trusses on a residential addition. The top section of the scaffold was 7 feet above the ground below and was not fully decked between the uprights and the guardrail supports exposing employees to falls in excess of 7 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about October 28, 2015, Employees were standing on the 8 inch wide CMU Concrete wall to set roof trusses on a new residential addition and were not protected by any means of Personal Fall Protection System, Guardrails, or safety nets, exposing the employees to a fall of over 10 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: A). On or about October 28, 2015 employees were setting roof trusses on a new residential addition and were utilizing a 12 foot aluminum extension ladder to gain access to the top of the CMU Wall. The ladder was not properly set with 3 feet of the ladder side rails extending above the wall top plate which did not present the employees with any means to steady themselves before descending the ladder from the wall top plate.
