29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    A). On or about October 29, 2015 Employees were working off the top section of a metal tubular scaffold system while setting roof trusses on a residential addition. The top section of the scaffold was 7 feet above the ground below and was not fully decked between the uprights and the guardrail supports exposing employees to falls in excess of 20 feet to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces and end rails were used as a means of access.  A). On or about October 29, 2015 employees were working on the top section of a metal tubular welded scaffold system and gained access to the top section by climbing the cross braces or side rail supports to gain access to the upper surface.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees:  A). Employees were using a 36" long Gorilla step work platform placed on top of the top scaffold section to increase the working height for the employee to reach the top plate of the wall while securing roof trusses to the top plate.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    A). On or about October 29, 2015 Employees working on the second story of a residential construction project were setting interior wood framed walls, and roof trusses, and were not protected by any means of personal fall protection, guardrails or safety nets exposing employee working along the outer footprint of the second floor to falls in excess of 12 to 20 feet.

On or about October 29, 2015 Employees were setting roof trusses along the 12 foot tall exterior wall and were not protected by any means of Personal Fall Arrest System, guardrails or safety nets, exposing them to falls in excess of 20 feet to the ground below.
