29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On or about October 29, 2015, and at times thereto, employees were using an electrical multi-conductor flexible cord that was not connected to a device or fitting as to prevent wires to pull directly from the twist lock terminal screws plug.  This condition exposed employees to electrical and fire hazards.
