29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about August 29, 2016 and at times prior thereto, the employer did not instruct employees to recognize the hazard of a multi-conductor flexible cord and plug strain relief device when not use effectively. This condition exposed employees to electrical shock and fire hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 29, 2015, and at times thereto, employees were using an electrical multi-conductor flexible cord that was not connected to a device or fitting as to prevent wires to pull directly from the twist lock terminal screws plug. This condition exposed employees to electrical and fire hazards.
