29 CFR 1910.261(b)(1): The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment:    (a) In the facility; During the time period from October 23, 2015 through October 27, 2015 equipment was not deenergized and locked out prior to performing cleaning, clearing jams and servicing on equipment including, but not limited to the following:   i.  cleaning and clearing jams on the wet lap machine;  ii. servicing, cleaning and clearing jams on the forward cleaners; and  iii. servicing and clearing jams on the course screen in the deinking process.

Cellu Tissue-CityForest, LLC dba Clearwater Paper-Ladysmith was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 279525, citation number 1, item number 1A and was affirmed as a final order on May 08, 2012, with respect to a workplace located at 1215 East Worden Avenue, Ladysmith, WI 54848.
