29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On October 29, 2015 at the jobsite located at or near 1320 Hilary Dr. in Liberty, MO, employees were exposed to an eye and face hazard in that they were using tools with the potential for eye injury without the use of eye protection.

29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  On October 29, 2015 at the jobsite located at or near 1320 Hilary Dr. in Liberty, MO, employees were exposed to a fire hazard in that the jobsite they were working on had more than 5 gallons of gas on site, but no fire extinguisher was within 50 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On October 29, 2015 at the jobsite located at or near 1320 Hilary Dr. in Liberty, MO, employees were exposed to a fall hazard in that they were working on a steep roof, approximately 20 feet above next lower level without an approved form of fall protection.
