(a) At the project site, a wall outlet receptacle box was not provided with a face cover plate and the exposed terminals were energized.  The box was located less than 12 inches from the workroom floor.  Employees were exposed to possibly coming in contact with exposed live parts and could receive an electrical burn and shock.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  (a) At the project site, employer did not provide the proper gloves such as, but not limited to, butyl or nitrile gloves for tile setters who mixed thin set mortar with water, CEG Part A epoxy grout color pigment and hardener to install tiles in a cardiology restroom.  Employees were exposed to possible skin irritation.
