29 CFR 1926.501(b)(10):    Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  At the Worksite:  110 Route 10 West, Whippany, NJ.  Roofing workers were exposed to falls of approximately 27 feet to the ground below as low-slope roof fall protection methods such as but not limited to a safety monitoring system were not in use when workers were applying the TPO Roofing System.                                               Violation occurred on or about 10/27/2015.

29 CFR 1926.1051(a):   Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a)  At the Worksite:  110 Route 10 West, Whippany, NJ.  Roofing workers were exposed to falls of up to 10 feet to the mezzanine roof below as they accessed the top roof using the frames of the tubular welded frame scaffolding which is not meant for climbing.                                           Violation occurred on or about 10/27/2015.
