29 CFR 1926.600(a)(6)(i): For lines rated 50kV or below, a minimum of 10 feet was not maintained between the energized line and the equipment. a) For employees dumping shell rock to grade a road in the orange groves with a 1999 Mack road tractor Model CH613, within the vicinity of 12010 NE Hwy 70, Arcadia, FL 34266. Employee suffered electrical shock and electrical burns when the dumping bed made contact with the 23kV power lines above while unloading shell rock on or about 10/19/2015 at 4:30 pm.

29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: a) For employees dumping shell rock to grade a road in the orange groves with a 1999 Mack road tractor Model CH613, within the vicinity of 12010 NE Hwy 70, Arcadia, FL 34266. Employee suffered electrical shock and electrical burns when the dumping bed made contact with the 23kV power lines above while unloading shell rock on or about 10/19/2015 at 4:30 pm.
