29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Metal Shed: On, or about, November 3, 2015, the following deficiencies were observed on the Mitsubishi Model FG-25 forktruck, serial number AF178-50551: A. The forktruck did not have an operable horn. B. The emergency brake did not work. C. The cover was missing from the wiring harness located in the steering column. D. A solid weather cover was provided that blocked visibility through the over head guard. E. The seat was worn, exposing the inner springs.
