29 CFR 1910.266(f)(1)(ii): The employer did not assure that each machine, including any machine provided by an employee, was inspected before initial use during each workshift:    On or about at 11/2/15 located at 2310 Blue Ridge Dr., McCaysville, GA:  The employer failed to have his employees inspect chipper (sr. no. 41690) before using for defects and damages. The warning decals were damaged and could not be read. Do not operate this machine unless all decals are legible, in place and operator thoroughly understands them.

Only properly and fully trained employees should operate the chipper.
