29 CFR 1904.29(b)(4): A form with equivalent information to the OSHA 301, Injury and Illness Incident Report, was not used by the employer to document each recordable injury and illness entered on the OSHA 300 Log:  (a) The employer did not maintain an equivalent OSHA 301 Incident Report that contained the same information as the OSHA form. During calendar year 2015, the employer recorded two injuries that resulted in days away from work and the equivalent form used did not contain essential information required by OSHA.
