29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:  a)  In the Facility, on or about November 3, 2015 - The employer did not ensure that employees were fit tested with the same respirator facepiece model that was used by the employee.  An employee designated as a burner was utilizing a 3M half mask respirator, but was fit tested with an MSA half mask respirator.  b)  In the Facility, on or about November 3, 2015 - The employer did not ensure that an employee designated as a burner, who wears a tight-fitting half mask respirator, was fit tested annually.
