29 CFR 1926.200(h)(1): 29 CFR 1926.200(h)(1):  Accident prevention tags were not used a temporary means of warning employees of an existing hazard, such as defective tools, equipment, etc.  a) Bldg 162, CLC Remodel, Mountain Home VA - On or about 11/3/15, a damaged GFCI was not tagged out preventing use.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  a) Bldg 162, CLC Remodel- Mountain Home VA - On or about 11/3/15, employees were exposed to tripping or stepping into holes when column holes were not securely covered.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
