29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Third floor, new residential construction located at 3030 Devoncroft Street, Smyrna, GA 30080: Employee was exposed to eye injuries while operating a Hitachi (Model NR83AS1) Framing Nail Gun without protective eyewear. The employee was exposed to struck-by hazards which included flying debris and fasteners while operating this equipment in close proximity to the eyes and face with no safety glasses or goggles.   (b) Truss laydown area, new residential construction located at 3030 Devoncroft Street, Smyrna, GA 30080: Employee was exposed to eye injuries while operating a PNEU Tools (Model SN2283H) Framing Nail Gun without protective eyewear. The employee was exposed to struck-by hazards which included flying debris and fasteners while operating this equipment in close proximity to the eyes and face with no safety glasses or goggles.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) Temporary Service, new residential construction located at 3030 Devoncroft Street, Smyrna, GA 30080: The employer failed to inspect all of the electrical cords and equipment powered by electric prior to the start of the work shift. The electrical cord attached to the ROLAIR Air Compressor (Model 5715K17-0045) had a missing ground pin, damaged strain relief, and a detached box connector which exposed the energized insulated electrical circuits.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Top plate, new residential construction located at 3030 Devoncroft Street, Smyrna, GA 30080: Employee was exposed to falls of approximately 28' to the lower level while engaged in framing activities on the top plate with no fall protection system in place.   Gwinnett Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 875983, citation number 1, item number 1 and was affirmed as a final order on 06/18/2013, with respect to a workplace located at 1207 Hillside Green Way, Powder Springs, Georgia 30127.
