29 CFR 1926.453(b)(2)(iv):   Employees did not always stand firmly on the floor of the basket, and did sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.  On 10/20/2013, employees from Dyer Construction Company of America, Inc. were working on a roof of a commercial building located at 1047 East Highway N in Wentzville, MO.  An employee was standing on the guard rails of the aerial lift basket while installing insulation on the edge of the roof, which was approximately twenty-five feet above the ground.  He did not always stand firmly on the floor of the basket while working.

29 CFR 1926.453(b)(2)(v):   A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  On 10/20/2013, employees from Dyer Construction Company of America, Inc. were working on a roof of a commercial building located at 1047 East Highway N in Wentzville, MO.  An employee was installing insulation on the edge of a roof of a commercial building in an aerial lift, Genie S-45/Serial #5970, which was approximately twenty-five feet above the ground.  The employee was wearing a body harness, but was not attached with the lanyard to the boom or basket.

29 CFR 1926.501(b)(10):   "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   On 10/20/2013, employees from Dyer Construction Company of America, Inc. were working on a roof of a commercial building located at 1047 East Highway N in Wentzville, MO.  Two foremen and two employees were installing insulation on the roof, which was approximately twenty-five feet above the ground without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
