29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At a residential apartment construction site, three employees performing remodeling work using circular saws and air nail guns were not provided eye protection. The employees were exposed to eye injuries.

29 CFR 1926.304(i)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.  At a residential apartment construction site, employees performing remodeling work were using a Porter Cable Table Saw, Model PCB222TS, Serial Number 034694 saw that was not guarded by a hood exposing the blade. Employees were exposed to amputation hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a residential apartment construction site, employees performing remodeling activities were exposed to an unprotected fall hazard of approximately 12 feet to the ground below.
