29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:  (a) On or about 11/09/15 - 2635 5th Ave N, Suite 115, Bessemer, AL, blasting abrasive material was allowed to accumulate throughout the plant, including but no limited to, inside and outside of the blasting booth area and at the pipe rolling area, exposing employees to a slipping hazard.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) On or about 11/09/15 - at the Blasting Booth, a medical evaluation was not provided for employees required to wear a Clemco Apollo supplied air respirator.  Liberty Coating Alabama, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 816941, citation number 1, item number 2 and was affirmed as a final order on 04/12/13 with respect to a workplace located at 2625 5th Avenue North, Bessemer, AL  35020.
