Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On or about November 10, 2015, during molding activities, the employer did not ensure that personal protective equipment for the forearms were used when employees were exposed to burn hazards.

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: a) On or about November 10, 2015, during molding activities, employees were exposed to burn hazards when the employer did not ensure an evaluation was completed for employees wearing the appropriate heat resistant gloves relative to the job tasks that they performed.
