29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:    At the above addressed jobsite, an employee working from an aerial lift did not attach a lanyard from his body harness to the boom or basket. The employee was thereby exposed to hazards associated with a fall of approximately thirteen (13) feet.    There is no abatement certification or documentation required for this item.
