29 CFR 1926.502(d)(15)(i): 29 CFR 1926.502(d)(15)(I):  Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two:     a) 10 Old Wernersville Rd, Sinking Spring PA - Two roofers performing roofing operations located at 11 feet - 8 inches above the ground were connected to the same Werner Upgear roof anchor as part of their fall protection systems when the anchor was only rated for 5,0000 pounds, on or about November 13, 2015.    No abatement certification or documentation required.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  a) 10 Old Wernersville Rd, Sinking Spring PA - Employee(s) were exposed to fall hazards while performing roofing work on a residential house. The employer failed to instruct each employee in the recognition and avoidance of unsafe conditions such as the safe use of fall protection equipment and the safe use of ladders, on or about November 13, 2015.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:    a) 10 Old Wernersville Road, Sinking Spring PA - A 20 foot metal extension ladder was used to gain access to the roof and the side rails did not extend at least 3 feet above the landing surface, exposing employees to an 11 foot - 8 inch fall hazard to the ground below, on or about November 13, 2015.    No abatement certification or documentation required.
