29 CFR 1904.8 (a): Employee(s) names were not removed from the 300 Log when employee(s) experienced needle stick and sharps injuries:  a) Richmond University Medical Center/ NY site: On OSHA 300 logs for 2016 and previous years, instead of using the "Privacy Case" designation, employees names were entered on the 300 log form for sustaining needle stick injuries . Instances include but not limited to entries in 2016 involving finger stick incidents while employees were involved in operations such as cleaning of craniotomy tray, transfer of blood, etc.   Observed on or about 04/26/2016.

29 CFR 1910.1030(h)(5)(i):  The sharps injury log shall be recorded and maintained in such manner as to protect the confidentiality of the injured employee:  a) Richmond University Medical Center/ NY site: Employees' names were entered on the sharps injury log. Designation such as "Privacy Case" to protect the confidentiality of the injured employees were not used. Instances include, but are not limited to entries in 2016 sharps injury log involving finger stick incidents during tasks such as cleaning of craniotomy tray, transfer of blood, etc.   Observed on or about 04/26/2016.

29 CFR 1910.1030(c)(1)(iv)(B): The review and update of the exposure control plan did not document annually consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure:   a) Richmond University Medical Center/ NY site: The exposure control plan was not updated to include and document the evaluation and implementation of appropriate commercially available and effective safer medical devices  such as but not limited to retractable winged steel needles, retracting lancets etc. currently being used at the facility.     Observed on or about 04/26/2016.
