29 CPR 1910.178(l)(1)(ii) Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator has successfully completed the training required by this paragraph (1), except as permitted by paragraph (1)(5): (Construction Reference: 1926.602(d).) a) Jobsite: On or about November 13, 2015, the employer did not ensure that forklift operators, were trained in the safe operations of the forklift, exposing employees to hazards such as but not limited to being struck by moving objects or falling loads and crushed by forklift tipping.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59) a) Jobsite: On or about November 13, 2015, temporary employees were not trained on chemical hazards associated with Iron Oxide, Copper Fumes, and/or Manganese Fumes present in their work area while employees were welding.
