29 CFR 1910.244(a)(2)(iii): Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack:    On or about November 9, 2015 and in times prior thereto, an employee was exposed to crushing and struck by hazards while replacing the brake pads on an automobile. The vehicle was elevated on jacks without blocking, cribbing, or other means to secure it.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  On or about November 9, 2015, an on the job hospitalization was not reported to OSHA within 24 hours.
