29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a)19-21 Belvediere St., Holyoke, MA: On or about November 6, 2015, several employees were roofing this residence without any fall protection more than 20 feet above the ground below.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.    a)19-21 Belvediere St., Holyoke, MA: On or about November 6, 2015, an employee roofing this residence was wearing FP that was attached at the wearer's waist, not in the center of their back near shoulder level, or above the wearer's head, exposed to a fall of more than 20 feet to the ground below.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
