29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. On or about 11/12/2015, at the job site - personal fall protection was not attached to the basket anchor point by one employee and the other employees were not using personal fall protection while installing metal frame from an aerial lift, exposing the worker to a 4 feet 6 inch fall hazard.
