29 CFR 1926.34(c): Means of egress was not continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency: a) The hatch door entrance to the cellar was blocked by a fixed scaffold reducing the door opening to approx 15 inches wide. Employees had to squeeze through the opening and were exposed to obstructed egress in the event of fire, smoke, or other emergency. Location: 653 5th Ave, New York, N.Y. 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: Employees were exposed to unprotected floor holes at the following instances: a) A 6 ft wide floor hole was left unprotected; 2fl Townhouse room off the stairwell. b) A 15 inch wide floor hole was unprotected at the material hoist entrance due to the hoist floor plate/ ramp being left up. c) Pipe riser shaft 2 ft wide opening was unprotected; Townhouse 2fl. d) A barricade/ guardrail was not provided at the elevator pit which was approx 7 ft deep; Townhouse cellar.
