29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):      a)  On November 18, 2015, and at times thereafter, the employer failed to determine the presence of hazards in Area 3, where the employer was made aware of Dieldrin-contaminated soil.  The employer did not determine whether personal protective equipment was necessary for employees working in the area.

29 CFR 1910.1200(h)(2)(ii): The employer did not provide information to the employees on operations in their work area where hazardous chemicals were present: (Construction Reference: 1926.59):      a)  On November 18, 2015, and times thereafter, employees were assigned to work in areas where soil contaminated with the pesticide Dieldrin was present and information was not provided to the employees about the conditions and the potential for health hazards.
