29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about November 19, 2015, at the above addressed jobsite, employees were working on a commercial roof around energized power lines exposing the employees to electrocution hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about November 19, 2015 at the above address employees were performing roofing work on a commercial roof. Employees were not utilizing fall protection and were exposed to a fall hazard of approximately 9.5 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
