29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) FMH Material Handling Solutions, Inc, 5151 Bannock Street, Suite 4, Denver, CO 80216: On or before September 28, 2015 the employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely. An employee lost control of his forklift when traveling down a ramp at a customer's site. The employee did not receive training in a stand-up Crown Forklift Model RC3020-35, Serial No. 12276757. This condition resulted in an amputation of part of this employee's left foot.

29 CFR 1910.178(l)(3)(i): 29 CFR 1910.178(l)(3)(I): Powered industrial truck operators had not received initial training regarding truck-related topics: a) FMH Material Handling Solutions, Inc, 5151 Bannock Street, Suite 4, Denver, CO 80216: On or before September 28, 2015 the employer did not ensure that employees received initial training in truck related topics identified through [A] to [M] for stand-up lifts. An employee lost control of his forklift when traveling down a ramp at a customer's site. This employee did not receive training in a stand-up Crown Forklift Model RC3020-35, Serial No. 12276757. This condition resulted in an amputation of part of an employee's foot. Abatement note: Truck-related topics include: [A] Operating Instructions, warnings, and precautions for the types of truck the operator will be authorized to operate; [B] Differences between the truck and the automobile; [C] Truck controls and instrumentation: where they are located, what they do, and how they work; [D] Engine or motor operation; [E] Steering and maneuvering; [F] Visibility (including restrictions due to loading); [G] Fork and attachment adaption, operation, and use limitations; [H] Vehicle capacity; [I] Vehicle stability; [J] Any vehicle inspection and maintenance that the operator will be required to perform; [K] Refueling and/or charging and recharging of batteries; [L] Operating limitations; [M] Any other operating instructions, warnings, or precautions listed in the operators manual for the types of vehicle that the employee is being trained to operate.

An employee lost control of a forklift when traveling down a ramp at a customer's site. This employee did not receive training on the operation of a stand-up Crown Forklift Model RC3020-35, Serial No. 12276757. This condition resulted in an amputation of part of an employee's foot.
