29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  On or about 11/16/2015, Columbus Ave.  a) The employees used a 100' extension cord to power the Makita skilsaw. They were not using a ground fault circuit interrupter.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  On or about 11/16/2015, Columbus Ave.  a) The employees used the 16' aluminum extension ladder to gain access to and from the work area. The ladder was not secured to prevent displacement.
