The unsafe conditions included:    1)  The volume of traffic;    2)  The temporary lane closures of one lane of a two lane road using flaggers;     3)  The movement of personnel, equipment, materials, and supplies in the work zone;    4)  The duration of work including working at night; and    5)  The fall hazards associated with open manholes.    b)  Maverick Pump Services, LLC at 23rd Ave. and 2nd Street, Greeley, CO 80631:  On or before November 4, 2015, the employer did not instruct each employee on the exposure to the biological hazards associated with sewer work.        Conditions exposed employees to the hazard of being struck-by motor vehicles, fall hazards, and biological hazards.    Abatement note:  The instruction failed to include the movement of the employees, equipment, and materials in the work zone;  the duration of work;  the traffic control plan;  and the communication with the flaggers.

The sewer bypass company was connecting pipes and a pump to the existing sewer line that was accessed through the manhole located at this intersection.   The sunset was at 4:52 pm, and the crew worked until at least 9:00 pm.  The work area was not provided with at least 5 foot candles of illumination.  Condition exposed employees to the hazard of being struck by motor vehicles, resulting in serious bodily injury.

The   The employer used a John Deere Mini Excavator Model 50 with this rigging to hoist the pump, the fuser, and piping from trailers.  The 3 inch wide polyester rigging, manufactured by Bishop Lifting Products, Inc. model 03EE2PX-111, serial number 311178, was damaged beyond its warning markers.  Condition exposed employees to the hazard of being struck by the swinging and falling materials and equipment.
